Follow your pattern as directed until it comes up to the part for you to change colors.

Complete your final stitch in your row, and then break the first color of yarn.

Take your new yarn color, make a loop, place it over your hook and pull it through. Pull your first color tighter to secure your second color in place.

Turn your work, take your loose ends and place them over your stitches.

As you work the next stitches that your pattern calls for, you are going to work around those ends and through the stitch beneath them to trap them into place.
